REGISTRATION FORM AND INSTRUCTIONS

GUIDELINES:
1. The registration form is comprised of two (2) pages. Download the
form and print both the form and the instructions. Read the
instructions carefully before filling out the form.
2. Although the instructions are quite thorough, we ask that you
consider the following;
a. Please print legibly.
b. If you receive your mail at a Post Office Box, Highway Contract or
Rural Route or through General Delivery, your home (residence) address
of your residence is required. A street or description of the
location, zip code or city will be helpful.
c. If you have previously submitted documentation to the Department of
Hawaiian Home Lands (DHHL), or the Office of Hawaiian Affairs (OHA)
for either the Native Hawaiian Registry or Operation Ohana Programs,
please enter your respective numbers. Also, if you will authorize DHHL
or OHA to release your information, please fix your initials in the
space provided.
d. If you verify your ancestry by selecting any of the authorized
certificates, i.e., Birth, Marriage or Death, please insure that a
photocopy is attached to your form when submitting your documents to
Hawai'i Maoli.
HINT: A photocopy of your birth certificate will greatly expedite the
verification process.
The above guidelines are some of the most important that we can
identify to help you in processing your application successfully. We
also hope you will keep us in mind should a change of address or
contact number occur. You may write or call us with your changes at
(808) 394-0050, between the hours of 9:00 am - 3:00 pm, Monday thru
Friday. Provisions for accepting this data on-line are being planned
as well.
